Confidentiality
Medical records/information is released only with your consent, except when required by law.

Reception Area
Please bring only one person to accompany you on your office visit. This will allow room in our reception area for all patients.

Registration
During your first appointment you will be asked to complete forms which will ask for such information as your name, address, social security number, insurance information, and past health history. Please bring this information with you.

Appointments
To provide the best possible care, we prefer to see our patients on an appointment basis. Please call us in advance so that a special time can be reserved for you. If you have an emergency, we will do our best to accommodate you. However, delays may occur. If you are unable to keep an appointment, please call us as soon as possible to reschedule.
