On Your First Visit

For your initial visit, please plan to arrive 15 minutes early to allow time for initial registration and to provide information on your medical history.

Please bring the following with you:

• Current Medications & Prescriptions (with allergy history)
• Medical Records (give to receptionist when checking in)
• Insurance, Medicare or Medicaid Cards
• Any questions you may want to ask the doctor

Managed Care Referrals
If you have been referred to us by an HMO physician, you must bring a
referral slip with you to your first appointment.
